Health nurses or masters level LMHC/LPC who are ready to leave

growth!Our Case Manager role allows you to utilize your clinical
experience to develop a plan for meeting health care needs of the
patient, while continuing to professionally grow in health care

Responsibilities
- Demonstrate knowledge and application of the nursing (or
LMHC/LPC) process as it applies to case management.
- Identify problems while assessing the patient's medical and
psychosocial status, including environmental, financial and
cultural influences.
- Collaborate with physician(s), provider(s), and patient/family
and payers to facilitate quality, timely and efficient care across
the healthcare continuum.
- Improve the health outcomes and quality of life with reduced
complications and costs for persons with chronic health
conditions.
- Promote patient self-management of chronic health conditions
through individual assessment and education.
- Demonstrate ability to communicate effectively with insurers,
physicians, patient/family, and other healthcare providers,
including ability to negotiate with assertiveness.
- Communicate in a calm, professional, and effective manner.
- Provide good customer relations by promoting goodwill and
understanding between claimant, payer, providers and professional
staff at all times.PM21Requirements:
- Hines and Associates ONLY hires licensed/certified medical
professionals with an unrestricted license/certification in the
state or in a state that has licensure reciprocation with the state
of the office location the employee is working in.
- Must be an RN.
- Successful completion of case management orientation
program.
- Minimum of 2 years in a full-time acute inpatient setting or
specialty area (i.e., emergency department, inpatient BH (including
float), or treatment program)
- Excellent communication skills
- Basic typing/computer knowledge with minimum keyboarding speed
of 35WPM.
- Previous case management experience helpful but not
required.
- Previous case management or insurance industry experience
