Job Description:
A company is looking for a Health Coach Consultant.
Key Responsibilities
Utilizes motivational interviewing skills to engage members and
assess their health needs
Applies clinical judgment to reduce risk factors and address
complex health issues
Collaborates with care teams and escalates member needs as per
guidelines
Required Qualifications
5+ years' experience as a Registered Nurse
1+ years of nursing experience with patients having Diabetes,
Cardiac Disease, ICU, CCU, Home Health, or Pulmonary Disease
1+ years of electronic documentation experience using multiple
computer screens
Active and unrestricted RN licensure in the state of residence
Associates degree or diploma required; Bachelor's preferred
